Personal Health Advocate

The UNC Personal Health Advocate Program is a new benefit that provides support and management of health care needs for eligible participants.

Members will receive access to their own personal health advocate team of trained nurses, pharmacists, dietitians, social workers, and care coordinators to assist them with their care needs and help them understand and manage their medications.

Eligibility

Participants are invited to join the Personal Health Advocate Program based on their individual care needs. If you or your enrolled dependents qualify, a Personal Health Advocate will contact you directly via telephone or letter.

  • Assistance with your care needs and help understanding and managing medications
  • Coordination and communication with your physicians and other health care providers
  • Help getting timely access to UNC Health Care physicians
  • Support after an emergency room visit or discharge from a hospital
  • Access to your Personal Health Advocate via telephone

Enrollment is by invitation only to members with specific conditions or need for extra support such as those taking many medications or members with multiple disease states, cancer, autoimmune disease, recent hospital discharges, or multiple ED visits.

No, but if you are suffering from an illness that causes you to access multiple physicians, be admitted to the hospital, or take more than ten prescription medications or certain medications, you may be eligible for the program.

We are here to support you to achieve optimum health, based on your individual needs. A review of your claims history and health information suggests you could benefit from additional resources to help you manage your medical condition.

This is a benefit of your health insurance plan and there is no additional cost to you.

No, you may see any health care providers you choose and your Personal Health Advocate will work directly with your providers whether or not they are part of UNCHC. If you do not have a primary care physician, or would like to switch to a new one, we will help you with that.

This program is a valuable service offered as part of your insurance benefit.  Some may receive a financial benefit for participation. All that join will have access to valuable care coordination benefits.

Unlike other programs, we will support your current treatment plan by coordinating directly with your physicians, the hospital or other health care providers that you may benefit from seeing through comprehensive care management tailored to meet your needs. If you don’t currently have a treatment plan or access to appropriate providers, we can help you with these.
