New Patients
Thank you for your interest in our practice. We are excited to provide a medical home for you and your family! Please review the following information to facilitate care with our practice.
First, call our office at 919-361-2644 and furnish us the following information:
- Patient's legal name
- Patient's address and telephone number
- Patient's valid insurance information
- Patient's date of birth
- Patient's referring physician
- Emergency contact
- Financially responsible person including legal name, address, telephone number.
If you presently have health insurance, you are expected to bring your current insurance card(s) with you at the time of your visit. If you are not able to bring your insurance card with you and we are not able to verify your information, we may need to reschedule your appointment.
Please bring a list of your current medications with you. If lab work is performed during your visit, you will receive a separate bill from UNC McClendon Labs.
