Please call Release of Medical Information at (919) 966-2336 for the fee schedule. | Can my medical records be mailed to me? Yes. Copies of medical records can be mailed to your home address. | What is the turnaround time for completing my request for copies of my medical record? The turn around time is five to seven business days upon receipt of request. | Where do I go to pick up a copy of my medical record? The Medical Information Management Department maintains all UNC Hospitals records. This includes N.C. Memorial Hospital, N.C. Children's Hospital, N.C. Women's Hospital, N.C. Neurosciences Hospital, the Ambulatory Care Center and the Family Practice Center. | How do I request an amendment to my medical record? You may request an amendment of protected health information (PHI) about you by completing the Request for Correction/Amendment form and mailing or faxing it to the Medical Information Management Deptartment Release of Medical Information Section: Release of Medical Information Medical Information Management Dept. UNC Hospitals 101 Manning Drive Chapel Hill, N.C. 27514
Fax: (919) 966-6295 UNC Health Care must act on your request for an amendment no later than 60 days after receipt of such a request. If UNC Health Care is unable to act on the amendment within the required 60-day time limit, UNC Health Care may extend the time for such action by no more than 30 days, provided that: | - UNC Health Care Release of Medical Information staff provides you with a written statement of the reasons for the delay, and the date by which action on the request will be completed; and
- UNC Health Care may have only one such extension of time for action on a request for an amendment.
