Referrals
When a patient has a life-limiting illness, it is appropriate to
discuss all available options for care including hospice
services. The sooner the UNC Hospice team can begin to help
the patient and family, the more effective our care will be in
improving the quality of life and decreasing the physical, emotional
and financial burdens. Hospice referrals are accepted from any
source. After receiving a physicians order for hospice
evaluation, our admission staff will visit to assess the needs of the
patient and family.

Referrals are taken 24 hours a day. After business hours,
referrals will be accepted by our on-call staff. The following
information would be helpful at the time of admission:
- Patient name, address and phone number
- Diagnosis, social security number and date of birth
- Attending physician’s name and phone number
- Insurance information
- Caregiver or family contact name and phone number
