MetLife Life Insurance – Domestic Partner Enrollment
September 1 to September 30, 2008
The UNC Health Care Insurance and Benefits Committee is pleased to announce that beginning September 1, 2008, UNC Health Care employees may choose to cover Domestic Partners under the MetLife Group Term Life Insurance policy. The amount of coverage on Domestic Partners is $25,000.
Current MetLife Life Insurance participants who add Domestic Partner coverage to their policies between September 1, 2008 and September 30, 2008 will have the Domestic Partner coverage automatically approved.
Employees not currently participating may enroll in coverage at any time by completing the enrollment application and submitting a Statement of Health for the employee and each dependent to be covered. Coverage is subject to approval by MetLife Underwriting.
Eligibility Criteria:
- Employee must be a regular (non-temporary) UNC Health Care employee working 20 or more hours per week.
- Domestic Partner may be of opposite or same gender as employee
- Domestic Partner must share residence with employee
- Domestic Partner and employee must have joint responsibility for basic financial obligations
- Enrollment application must be accompanied by a signed and notarized affidavit confirming the nature of the Domestic Partner relationship
