North Carolina State Retirement System Death Benefit
UNC Health Care employees who are regular, working 30 hours or more per week, contributing to the Teachers' and State Employees' Retirement System (TSERS), and who have a year or more of creditable state service are eligible to receive the State of North Carolina Retirement System Death Benefit. This benefit pays an employee's designated beneficiary from a minimum of $25,000 to a maximum of $50,000.

MetLife Group Term Life Insurance
UNC Health Care offers group "term" life insurance coverage under Metropolitan Life (MetLife). This plan is administered by UNC Health Care and is available to all regular employees who work 20 hours or more per week. Premiums are deducted on a post-tax basis.
This benefit is effective the 1st day of the 2nd month after an employee's date of hire. Or the 1st day of the month after underwriting approval (late enrollments only).
The MetLife plan allows employees to insure themselves for up to five times their salary (up to $625,000). There are four available coverage types: Employee Only, Employee & Children, Employee & Spouse, and Employee & Family. The MetLife plan allows an employee to cover their dependent spouse for up to $25,000 and cover their dependent children at $10,000 each.
Domestic Partners: UNC Health Care employees may choose to cover same-sex or opposite-sex Domestic Partners under the MetLife Group Term Life Insurance policy. The amount of coverage on Domestic Partners is $25,000. The enrollment application must be accompanied by a signed and notarized affidavit confirming the nature of the Domestic Partner relationship.
The MetLife plan also offers accidental death and dismemberment insurance to employees who are enrolled in the life insurance plan. Coverage is only available for the employee and not dependents. AD&D coverage levels will equal the amount of life insurance coverage on the employee.

Unum Interest Sensitive Whole
Life
UNC Health Care offers "whole" life insurance to all regular employees who work 20 or more hours per week. The plan is administered by Unum Provident. New hires have 30 days from date hired to enroll in this insurance plan. Premiums for this life insurance benefit are deducted on a post-tax basis.

NC Flex Accidental Death & Dismemberment Insurance
UNC Health Care offers employees accidental death & dismemberment insurance (AD&D) coverage under the North Carolina Flexible Benefits Program (NC Flex). As of January 1, 2009, this plan is insured by A.C. Newman Company. The NC Flex AD&D Insurance is available to all regular employees who work 20 or more hours per week.
The effective date of this benefit is the 1st day of the month after the date of hire or eligibility change. For employees enrolling during the Annual Enrollment Period (October 3-31, 2011), the effective date of coverage will be January 1, 2012. Enrollment must be completed online - no paper forms will be accepted.
There are two components of the NCFlex AD&D insurance: Core AD&D coverage and Voluntary AD&D coverage, and you may choose to enroll in either or both. Both plans pay a benefit if you suffer a loss as the result of an accident while insured, or if you suffer certain disabling injuries while insured by the plan. The coverage is effective 24 hours a day and includes accidents on and off the job.
The Core AD&D benefit provides a Principal Sum for Employee Only in the amount of $10,000 at no cost to you, but you must enroll in the Core AD&D to receive this benefit. This coverage can be elected even if Voluntary AD&D coverage is not elected. The Voluntary AD&D Insurance offers two different coverage types: Employee Only and Employee Family. Under Employee Only coverage, employees are covered for 100% of the principal amount for which they are insured when death occurs due to an accident. Under Employee Family coverage, dependent spouses are covered for 60% of the principal sum when death occurs, and dependent children are covered for 15% of the principal sum when death occurs. NC Flex Voluntary Group Term Life Insurance
Voluntary Group Term Life Insurance is available to eligible UNC Health Care employees through the North Carolina Flexible Benefits Program (NC Flex). The plan is offered by ING Employee Benefits and is underwritten by ReliaStar Life Insurance Company. Voluntary Group Term Life Insurance pays a benefit to your beneficiary(ies) if you die while covered under the policy. Please note: there is no accumulated cash value.
The NC Flex voluntary group term life insurance is available to all regular employees who work 20 or more hours per week. If you enroll in this plan the first time it is offered, you may elect coverage up to $100,000 without providing evidence of insurability (EOI). Enrollment must be completed online.
As a new hire enrolling for $100,000 of coverage or less, the effective date of this benefit is the 1st day of the month after the date of hire or eligibility change. If your enrollment requires an EOI, coverage will be effective the first of the month on or following the date your EOI is approved by ReliaStar.
If you enroll for coverage during the Annual Enrollment period, and your EOI is accepted prior to January 1, your coverage will be effective January 1, 2012. If your EOI date of approval is after January 1, 2012, your coverage will be effective the first of the month following the date your EOI is approved by ReliaStar. *TAX ISSUE: Premiums for life insurance are deducted from your pay on a pre-tax basis for coverage amounts up to $50,000. Premiums for life insurance amounts in excess of $50,000 are paid on a post-tax basis. The premium for coverage in excess of $50,000 is added back into your pay and taxed accordingly. This occurrence is called “imputed income.” While imputed income occurs, the covered employee still benefits from lower group term rates through the NC Flex program.
