Primary Care Improvement Collaborative (PCIC)
The Primary Care Improvement Collaborative (PCIC) is a Physician driven collaborative joining practice and support staff across the UNCHCS in a learning & sharing forum dedicated to improving quality for patients in the domains of clinical quality, access, and patient engagement. The Collaborative has a formal governance structure inclusive of developing and endorsing meaningful measures addressing each quality domain. Performance goals and benchmarks are established to drive performance improvement and reduce variation across the UNCHCS. It is the principal hub for practice transformation aimed at enhancing the patient experience and improving the health of North Carolinians.
Patient Centered Medical Home (PCMH) recognition has been a cornerstone of UNC’s primary care practice transformation since 2012. In fiscal year 2018, UNC Health Care consolidated several quality improvement initiatives to create a new PCMH framework that is a system driven strategy designed to further transform primary care. The program streamlines patient centered practice transformation elements under four pillars: workflow / population health, quality measurement/ sharing best practices, practice collaboration, and patient/clinical staff safety.
Directs the Center for Medicare and Medicaid Services' (CMS) complex Merit-Based Incentive Payment System (MIPS) and Medicaid Meaningful Use (MU) programs for the UNC Health Care System (UNCHCS). Through close collaboration with UNCHCS entity and clinical leadership, coaching, population health and information services, our Government Programs team continually reviews and analyzes the evolving landscape of CMS policy and rulemaking in order to ensure each applicable UNCHCS entity, or group, achieves the highest potential annual MIPS score, and that UNCHCS Eligible Professionals receive applicable Medicaid MU incentives.
