Primary Care Improvement Collaborative (PCIC)
UNC Health Care provides comprehensive primary care to a large population of North Carolinians through a variety of locations, both on campus at the health care system complex and in our local communities. We have a long history of innovation including new models of care, quality improvement, and research. As a leading healthcare system we are committed to providing patient care and quality outcomes at the highest level.
Established in 2013, the Primary Care Improvement Collaborative (PCIC) is the hub for practice transformation aimed at enhancing our patients’ experience and improving quality outcomes.
Primary care practice representatives from across the UNC Health Care System have joined together as a Primary Care Improvement Collaborative (PCIC) to facilitate shared learning and accountability. Working together we believe we will advance quicker to meet organizational improvement targets, and spread effective strategies to our UNC primary care practices.
The PCIC executive team provides overall leadership and direction for the collaborative. Members of the Practice Quality & Innovation team provide:
- Project management support, including PCIC meeting management and communication
- Analytic support, including creation of a monthly PCIC dashboard
- Quality coaching, including supporting practices improvement efforts using improvement methods such as Model for Improvement, PDCA cycles, and Lean Six Sigma
- Population health tools and implementation support
