How to Apply
Instructions for the 2015-2016 application process (except for PGY2 Drug Information/Safety and PGY1 Ambulatory Care):
STEP 1- All applicants to ASHP-accredited pharmacy practice residencies must participate in the match process sponsored by the National Matching Service. Applicants must submit match agreements to the National Matching Service to obtain a match number. Applications and deadlines can be found at the following link: www.natmatch.com/ashprmp/
Of note, for applicants to the Drug Information/Medication Safety PGY2 Residency Program please use the following link: DI/Safety application
STEP 2 – The UNC Hospitals will be participating in PhORCAS, the new ASHP residency online application tool. More information can be found here: http://www.ashp.org/phorcas
STEP 3 - A complete application will have the following information found through PhORCAS:
- General information about the applicant
- Cover letter
- Response to essay questions
- Curriculum Vitae
- Pharmacy School Transcript
- Three (3) references. Reference writers should complete the standardized Phorcas reference form only.
- Current photograph (optional)
Note – for those individuals applying to the 2-year MS / Health-System Pharmacy Administration residency program, all applicants need to complete a graduate school application (http://gradschool.unc.edu/admissions/applicant_dom.html))in addition to the residency application.
Note - for individuals applying to the Drug Information/Med Safety PGY2 residency program, please contact Ashley Pappas (email@example.com), RPD for the program, for application materials. This program is not participating in the PhorCAS process this year
Note - For individuals interested in the PGY1 Ambulatory Care Residency Program, the PhorCAS/WebAdMIT application is now available. Program code is 36050. Because of the delay in the application being available, the application deadline for this program has been extended to January 12th. (updated 1/6/15)
Application Deadline is January 1st.
Applicants: must submit all required materials as well as uploading a document with answers to UNC-specific essay questions (see supplemental information above).
